Recent research published in Cell Metabolism investigated the metabolic effects of three distinct diets in individuals with obesity, prediabetes, and fatty liver disease. The study compared a ketogenic diet, a Mediterranean diet, and a low-fat, plant-forward diet over a period of four to five months, with all groups achieving approximately 10% body weight loss.

The clinical trial enrolled 42 participants who were randomly assigned to one of the three diets. Each diet varied substantially in macronutrient distribution: the ketogenic diet provided 4% of calories from carbohydrates and 73% from fat, the Mediterranean diet supplied 50% carbohydrates and 35% fat, and the low-fat plant-forward diet included 70% carbohydrates and 15% fat. All food was provided by the research team, and participants received weekly dietitian support to maintain adherence.

Key Findings on Liver Fat and Metabolic Health

Despite similar weight loss across groups, the ketogenic diet resulted in a 67% average reduction in liver fat, compared with 45% reductions in both the Mediterranean and plant-forward groups. Improvements in insulin sensitivity were observed in all groups, but the ketogenic group exhibited the most pronounced hormonal changes, including increased glucagon and decreased insulin levels, which may contribute to the reduction in liver fat.

Notably, about half of the ketogenic diet participants no longer met the criteria for prediabetes after the intervention, compared to 29% in the Mediterranean group and 7% in the plant-forward group. The study also found no increase in blood fat or cholesterol levels in the ketogenic group despite the high fat intake.

Context and Considerations

While these findings suggest that carbohydrate restriction may offer metabolic benefits beyond weight loss alone, it is important to recognise that this is a controlled clinical trial with a relatively small sample size and short duration. Further research is needed to confirm long-term effects and safety.
